What is Scar Revision?

What should patients expect after Scar Revision

Each scar revision procedure is different and catered to the individual patient. Therefore, the types of incisions utilized as well as extent of swelling and bruising that occur after surgery will vary by patient. Every reconstructive patient will follow-up with Dr. Ho one week after surgery for suture removal. Each patient will then follow-up again at weeks to months out at regular intervals for re-evaluation. The down time for surgical scar revision is usually minimal; most patients can return to normal daily activities apart from heavy lifting, straining, and strenuous activity the day after surgery. Post-operative pain is usually mild and managed well with prescribed pain medication. The scars are typically well-healed by two weeks out from surgery but take up to six months or longer to mature and achieve their final appearance. Z-plasty is one scar revision technique that utilizes triangular flaps created and rotated along the scar to improve the appearance of the scar. It is often used to lengthen a contracted scar, to change the direction of the scar so that is oriented in the direction of the natural lines of the face, and to break up a prominent scar for better concealment. The 60-degree Z-plasty is the most common approach to Z-plasty and results in a 75% increase in scar length and 90-degree change in scar direction.

Every scar revision surgery is different and thus can have varying degrees of pain. Luckily, in the majority of these procedures, the pain is typically minimal and well-controlled by using over-the-counter or prescription pain medications such as Tylenol. The majority of pain is experienced during the first few days after surgery and tends to resolve quickly. 

The majority of patients will return to work and normal daily activities the day after surgery but should refrain from strenuous exercises, heavy lifting, and straining until cleared by their surgeon. Most scar revision surgeries have healed by 2 weeks, but the end result may take 6 to 12 months or more to be fully appreciated.

The purpose of scar revision is to provide a more natural, less visible scar, but no scar can be permanently erased. The techniques employed during surgery help to minimize the tension on the healing tissue and promote a more aesthetically appealing scar. The goals are to help reduce the appearance of hypertrophic and keloid scarring and improve the natural look of the affected area.

Minimizing tension and irritation to the scar is essential to its proper healing. Therefore, it is important to leave the area alone as much as possible. This includes refraining from putting any lotions, creams, or tapes on the incision unless instructed to do so by your surgeon. Protecting the scar from the damaging effects of UV light is important, and thus, using adequate sunscreen and wearing protective clothing is important during the healing process.
